Read the information below and answer questions 15-21.

Powell Associates

Employment Verification Information

Section A

All demographics and information pertaining to all employees of Powell Associates must be up-to-date and kept confidential. Employers are responsible for retaining this information. All employees must provide a full legal last name, first name, and middle initial. Provide all other names used, in the case of maiden name and marriage. Providing both an email address and telephone number are voluntary.

Section B

Employers have many things that they must complete. Firstly, they must verify that employee information is credible and accurate. Employers must examine the evidence of identity and employment authorization within the first two weeks of employment. In the case that the employer suspects wrongdoing or false information on the employee’s documentation, the appropriate offices must be contacted.

Section C

In the event that an employee has worked for Powell Associates in the past, employers must follow different protocol. Even in the case that the employee was an unpaid intern, he or she would still be considered as a rehire for the company. As such, the employee can skip the verification process and rely on past forms. If, however, names have been changed, the employee must be treated as a new employee in all cases.

Section D

It is the employee’s responsibility to complete and process a monetary charge for filing the paperwork. The form is not filed with a government agency, and thus, is subject to a fee. All employees can discuss payment and receipt with the appropriate supervisor.

Section E

The collection of this information comes with a due process of privacy and confidentiality. An employee can expect that the data shared with his or her employer will not be shared with third parties unless otherwise noted. If there are cases of a breach in this contract, it is imperative that the employer notify the appropriate Board of Directors of Powell Associates in order for prompt action to be taken.

Section F

Employees should note that the section entitled, “Next of Kin” is reserved for those selected individuals that should be contacted in the case of crisis or unexpected occurrence. While this is optional, employees are advised to include as much information as possible in order for Powell Associates to make appropriate decisions in the unlikely event of an emergency.

Section G

It is illegal to discriminate against any work-authorized individuals in hiring, discharge, recruitment, or referral. The refusal to hire an individual because of race, creed, or nationality is absolutely prohibited. If employees believe that they have been subjected to such treatment, it is imperative that they follow-up with the Board of Directors of Powell Associates.

Altius Offer Letter of Employment

Congratulations! Altius would like to offer you a Marketing and Outreach Summer Associate position for the Summer of 2020. Your resume was first-rate, and we believe your past experiences make you a unique and excellent candidate for this position.

Hours and Compensation

This internship is a part-time, temporary summer position. No benefits will be offered with this position. Your hourly pay rate will be $10.00 per hour for a maximum of 20 hours per week, unless a mutual agreement is reached to increase those hours.

You have agreed upon a start date of Tuesday, June 11, 2020 at 9:00 AM. The 13 week summer position will end on Friday, September 6, 2020. You will also be required to submit a semi-monthly timesheet. You will receive a semi-monthly paycheck with all local, state, and federal taxes deducted. Mileage, over and above commuting to the site from home, will be reimbursed at the federal rate when driving is required to perform your responsibilities.

Supervision and Responsibilities

Mr. Mark Mayright will serve as your immediate supervisor, but you will have some direction provided to you by Ms. Julie Newman, who is our Development Associate. General direction from other Altius team members are a possibility. In the near future, Ms. Newman and Mr. Mayright will discuss with you your responsibilities, expectations and summer projects in greater detail.

Next Steps

If the scope and terms are acceptable, please sign below and return a copy to the attention of Mr. Mark Mayright in the self-addressed envelope. Additionally, all new hires are requested to receive a medical evaluation and physical, in order to adhere to compliance for working on-site. This must be completed no later than one week after the agreed upon start date.
