You received a letter from your insurance company stating you had missed the quarterly payment of insurance premium. Though you have had sufficient amount of money in your bank account, yet this has happened. Escalate a complaint to your bank about this.

SAMPLE ANSWER
Dear Manager,
I write this letter to draw your attention about the blunder committed by your team. I have had a savings bank account number 659823548 with your branch since April 2008. Last week, I received a ‘Due Payment Notification’ from my insurance company, Bharati AXA, stating that the premium amount of $250.00 has been missed.
The moment the notification letter came to hand, I was taken aback. My savings bank account is linked with the term deposit worth $30,000.00. As I have a line of credit (overdraft facility) linked to account, this payment should have been processed successfully. It seems that the line of credit did not trigger timely, resulting in dishonoring the check.
This oversight has cost me a penalty of $10.00 plus a blot on my credit history. Therefore, I demand you to write an apology letter owning up to your mistake, reverse the penalty fees, and inform Bharati AXA for the same for removing any notation if so linked to the credit history system.
I hope to have a favorable reply or I would escalate this issue with Banking Ombudsman Department.
