You have an appointment with the bank and now you realized that you can’t make it for that. Write a letter to the manager.

Dear Sir,
I am Akshitha. I am writing this letter regarding the cancellation of my appointment at 10.00 a.m. on 10th December, as I am not able to attend. I sincerely apologize for this last-minute intimation.
I have booked this appointment for applying online banking for my mother’s account. I was intimated that the bank account holder should be accompanied by me for signing the application. As my mother is the account holder and she is not in a position to come with me to the bank due to her ill-health, I have to cancel my appointment.
I hereby request you to kindly excuse me and reschedule it for any day in the first week of January.I hope that she will be fully recovered by that time. I also want to know if the appointment can be booked online by myself. You can call me at 999999999 and inform me of the exact date. we have been your customers for more than five years and are satisfied with the services you offer.
